Registration Information

Registration for the 2008 Season has ended.

Parents! Your child must be 4 yrs old by July 31, 2008 to register- Please see age matrix below!

Information on where applications for the 2008 season can be picked up at Sport & Cycle and other locations to be determined closer to registration date.

Projected Fees for the 2008 Season
All ages Division 4:
$60 1st family player
$55 2nd family player
$50 3rd and consecutive family player

All ages Division 3:
$60 per player

For the love of the game of soccer HYSL does not charge parents a monthly fee to pay coaches for their kids playing D1 or D3 soccer. All board members and coaches give of their time and expertise freely for the good of the game.

Make check payable to HYSL (a $10 service charge will be placed on any returned checks- your child will be placed on a waiting list until check clears) Please do not send cash. THERE ARE NO REFUNDS! HYSL does not guarantee team placement, special requests, or the availability of a uniform for registrations received after May 31, 2008. Players will be placed on a wait list and notified of team placement, depending on availability. Checks will be returned by September 30, 2008 for those not placed on a team. Teams are drafted in July. You will be notified by August 15th of your child's placement. Please do not call on with team placement questions until after August 15th. Follow the registration form instructions carefully. ALL INCOMPLETE FORMS WILL BE RETURNED-WHICH MAY JEOPARDIZE YOUR CHILD'S PLACEMENT ON A TEAM!!

Registration Form
Please complete a separate form for each child. Be sure the parent/legal guardian signatures are in the two boxes marked "Important" and "Consent for Medical Treatment (minor)." Do not write in the box marked "Official Use Only." Do not fill out first white box that asks for league and club info. Begin filling out form in second white box area.

1st Time Registration If your child is registering with HYSL for the first time, attach a photocopy of the verification of birth date- must be a state seal certificate, passport, alien registration card, or Medical card. Birth certificates are required for first-time registration for ALL players, and at every registration for the Competitive Level. Hospital, baptism, or religious certificates WILL NOT BE ACCEPTED.

Note- Returning players at Division 4 level should already be verified and no birth date verification is required.

Scholarships
A limited number of scholarships are available for needy players. In order to qualify for a partial scholarship (half of fee), proof of need-for-assistance must be supplied with the scholarship application at the time of registration. To obtain a scholarship application, contact the Recreation Coordinator at the club level.
